A 10 x 6 register means that it is going to fit into an opening that's 10 x 6. So we measure the back, not the front. The front of a register is actually going to measure a little bit bigger, but we would still refer to it as a 10 x 6. Most often the dimensions are read as width x height. The exceptions to this rule are with floor registers. Floor register measurements are listed as height x width.
